FIBA National-Team-Declaration

To complete a FIBA underage player transfer, the following is required:

• Clear image of passport
• Name of club and federation they are leaving
• Name of club they are transferring to
• Residence permit (proof of address)
• School enrolment letter
• Club enrolment letter
• Parents consent letter
• Notice of explanation (reason for transfer)
• Acknowledgement of National Team Restriction (here)
